Dear Parents and Guardians,

According to the New Jersey Department of Health, CHILDREN NOW ELEVEN YEARS OLD AND ENTERING GRADE SIX ARE REQUIRED TO RECEIVE A BOOSTER DOSE OF THE DIPHTHERIA, TETANUS, AND PERTUSSIS (Tdap) VACCINE AS WELL AS ONE DOSE OF MENINGOCOCCAL VACCINE.

Please check with your family physician to make sure these new immunizations have been given or will be given before September 2017.  An exception is made for students who will turn eleven after September 1, as they must be eleven to receive these immunizations.

If your child will not be eleven years old by September 1, 2017, please get a letter from your child’s doctor indicating the date of their immunization appointment.  These immunizations are expected to be completed within 30 days of turning eleven.

A letter indicating medical delay or a hard copy of your child’s immunization record indicating completion of the 6th grade immunizations that is signed, stamped, or initialed by your family doctor must be sent to or dropped off at the Walter Hill School before September 2017 in order for your child NOT TO BE EXCLUDED THE FIRST DAY OF SCHOOL.

If you have already completed this requirement, thank you.

Thank you for your cooperation,

Lisa Davis, RN Walter Hill School Nurse
